What an RO plant for a poultry farm does, and does not do
RO lowers TDS and hardness, which is what stops the scale.
RO alone does not deal with bacteria. Put UV or chlorination after the plant and keep the drinker lines clean, or treated water picks up germs on its way to the birds.

How often do the membranes need changing?
It depends on the feed water, how well the pre-treatment is kept up and how many hours the plant runs. Rising TDS at the outlet is the usual sign.
